The Learning Center for the Deaf has offered its families both a fully remote learning model and a hybrid learning model to begin the 2020-2021 school year.

This form acknowledges it is my family’s preference is to continue with remote/distance learning even though The Learning Center for the Deaf has made a hybrid/in-person model available. 

I acknowledge that The Learning Center for the Deaf will honor parent/guardian choice as it pertains to the best learning model for their student(s);

I acknowledge that this decision was made without any influence from The Learning Center for the Deaf;

I acknowledge that a doctor’s note is not required to participate in remote learning instruction; 

I acknowledge that my family may choose to change our model of learning during the school year and can do so by contacting the school’s Principal. A minimum of 2 weeks should be expected to help transition my child(ren) from one model of learning to another.

 

My signature below indicates I give consent, understand, and agree to TLC’s remote learning to provide high quality educational opportunities to my child.
